Do I need an appointment?

No appointment is necessary, we are a walk in facility. You can call us and make an appointment if preferred.

Where are you located?

We have 3 locations in Dade and Broward Count located at:

  • 7911 NW 72nd Ave., Suite 111 Medley, FL 33166
  • 1460 Palm Ave. Pembroke Pines, FL 33025
  • 711 NW 21st Court Miami, FL 33125
What are your operating hours?
  • Medley hours from 7:30am to 6:00 pm, M–F. Saturdays from 8:00 am to 1:00 pm
  • Pines hours from 8:00 am to 4:30pm, M–F. 
  • Little Havana hours from 8:00 am to 4:30pm, M–F. 
Do you accept my insurance?
  • Occupational medical testing services are either paid by the employer, a third party or the patient. Work injuries are covered by your employer’s workers compensation insurance carrier.

What do I need to bring to my appointment:

Drug & alcohol testing:

  • Picture id
  • The order or authorization for your test

Physical Examinations:

  • Picture id
  • The order or authorization for your exam
  • Any pertinent office visit notes from your primary care doctor
  • Any recent diagnostic test results
  • Names and dosage of all medications
